Blended Case Manager

Vision Towards Peace Counseling Services seeks a compassionate, organized, and detail-oriented Case Manager to join our team of dedicated mental health clinicians focused on providing comprehensive, patient-centered care. The Blended Case Manager will coordinate services and support individuals with mental health challenges, ensuring they receive the appropriate care, resources, and guidance to achieve their treatment goals. The ideal candidate will have strong interpersonal skills, a deep understanding of mental health conditions, and a commitment to enhancing the well-being of patients.

Key Responsibilities

Client Intake & Assessment:

  • Conduct initial assessments to determine the mental health needs of new clients.
  • Develop individualized treatment plans in collaboration with clients and clinical staff, ensuring that clients’ needs, goals, and preferences are considered.
  • Monitor progress toward treatment goals and adjust care plans as necessary.

Care Coordination:

  • Facilitate referrals to external services (e.g., housing, employment, medical care, legal services) to address clients’ needs beyond mental health care.
  • Maintain close communication with clients to track their progress and address emerging needs.
  • Serve as the primary point of contact for clients, their families, and other professionals involved in their care.

Resource Management:

  • Assist clients in accessing community resources, public assistance programs, and financial support.
  • Provide education on available resources such as mental health crisis services, substance use treatment, and social support systems.

Advocacy:

  • Advocate for clients’ rights and needs within the clinic and with external service providers.
  • Ensure clients understand their treatment options and help them make informed decisions.

Documentation & Reporting:

  • Maintain accurate, timely, and confidential case files, progress notes, and client records in compliance with clinic policies and regulatory requirements.
  • Prepare reports for clinical reviews, case conferences, and team meetings, ensuring that all necessary documentation is current.

Collaboration:

  • Work closely with clinicians, therapists, psychiatrists, and other healthcare providers to coordinate holistic care.
  • Participate in regular team meetings to discuss case progress, treatment updates, and best practices.

Crisis Intervention:

  • Provide crisis intervention and support in coordination with mental health professionals, ensuring client safety and well-being.
  • Help clients develop coping strategies and emergency plans for managing mental health crises.

Ongoing Support:

  • Conduct regular follow-ups with clients to assess progress and provide continued support, particularly during transitions (e.g., inpatient care to outpatient treatment).
  • Support clients in achieving long-term mental health stability and independence.

Qualifications

  • Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or
  • Registered nurse or
  • A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years experience in direct contact with mental health clients or
  • A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person before April 1, 1989

Experience:

  • Minimum of 2 years of experience in case management or a related role, preferably within the mental health or social services field.
  • Experience working with individuals experiencing mental health conditions, substance use disorders, or other complex needs is highly preferred.
  • Mental health direct care experience is working directly with mental health service consumers (adults, children, or adolescents), providing services involving casework or case management, individual or group therapy, crisis intervention, early intervention, vocational training, residential care, or social rehabilitation in a mental health facility or in a facility or program that is publicly funded to provide services to individuals receiving mental health services, or in a nursing home, a juvenile justice agency, or a children and adolescent service agency.

❖ Proof of education (transcript) and evidence of years of experience are required.

Licenses/Certifications:

  • Certification in Case Management (CCM) or other relevant certifications is a plus.
  • CPR and First Aid certification is preferred but not required.

Skills & Abilities:

  • Strong interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
  • In-depth understanding of mental health conditions, treatment modalities, and resources available for individuals with mental health and social challenges.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organizational skills, with the ability to manage multiple cases simultaneously.
  • Compassionate, non-judgmental approach to client care.
  • Proficiency in electronic health records (EHR) and Microsoft Office Suite.

Work Environment:

  • This role involves both in-office and fieldwork, as you may need to accompany clients to appointments or community resources.
  • Some evening and weekend hours may be required to accommodate client needs.
